Our Story
Identity Outreach is dedicated to supporting opportunity youth and vulnerable populations in the community. Our founder/leader Jasmine Tucker, was inspired by her own experiences as a youth participating in community-based services. Understanding the profound impact of providing services in the home rather than institutions, Jasmine pursued her education, completing a bachelor's degree in criminal justice at IUI and currently pursuing her master's in social work at the same institution. Driven by her passion to make a difference, Jasmine spearheaded the creation of our organization.
Our Vision
Our vision is to create a community where every youth and family feels seen, safe, and supported as they build the life they deserve. Guided by our core pillars of Identity, Safety, Growth, Empowerment, and Connection, we aim to break generational cycles, strengthen families, and prepare the next generation with the skills, mindset, and stability to thrive. Through intentional care, authentic relationships, and real-life tools, Identity Outreach will continue leading families toward lasting change, purpose, and legacy.
Our Mission
Identity Outreach exists to empower youth and families by providing safe, intentional spaces where healing, growth, and stability are possible. We meet people exactly where they are without judgment and help them build the skills, mindset, and confidence needed to move forward. Through supportive relationships, real-life guidance, and culturally aware care, we help individuals rediscover their identity, strengthen their families, and create a future they can be proud of.
